PSYCHIC ENTERTAINER FOR PARTIES AND EVENTS
DISCLAIMER FOR ENTERTAINMENT EVENT AGREEMENTS
1. Client/s agrees and acknowledges that SarahDawn provides Psychic Services strictly as a Party and/or Event Entertainer. Client agrees that SarahDawn's consultations are for entertainment purposes only and Client accepts full responsibility on their part in requesting her service.
2. Client/s agrees and acknowledges that Client and Client's Guests are aware that, as the service provided by SarahDawn is strictly for entertainment, the Client/s and their Guests will at no time hold SarahDawn and her associates legally liable if and when Client/s and/or any of their Guests misconstrues and/or misuses the information given them to their own detriment in any form and or manner.
3. Client/s agrees that in the event of any disputes, claims and/or controversies between our company and Client, said dispute will be submitted to and settled by arbitration in accordance with the American Arbitration Association and the Federal Arbitration Act, and remedy or relief awarded to the injured party shall not exceed the actual dollar amount paid for the original services, attorney fees incurred and other charges deemed necessary in this legal proceeding.
4. The above herein SarahDawn company's Disclosure and Disclaimer shall be provided to all Clients either via fax or email upon request.
5. Client/s agrees and accepts with full responsibility to the herein terms and conditions by affixing their signature below and dating this agreement at the time of said booking for the services of SarahDawn psychic entertainment services.
